Nursing personnel are involved with assisting the Community Liaison Director in meeting the needs of the community through:

Case Managers
Case managers facilitate collaborative care between patients, the family support system, the bedside nurse, physicians, and other ancillary and community resources. They identify patients requiring care management because of an unpredictable course of illness, frequent or repeat hospital admissions, increased socioeconomic risk, or special financial need. They work with the patient from the time of admission to plan for special discharge needs.
Licensed Clinical Social Worker
Our Licensed Clinical Social Worker [LCSW] works with patients and family members to meet identified psychosocial needs that are identified as a result of hospitalization, the course of illness, or in response to socioeconomic issues. An important member of the health care team, the LCSW can be contacted Monday through Friday.
